Coronavirus Health Self-Checklist

Before your visit to our office, please ask yourself the questions below. If you answer YES to any of them, please reschedule the appointment.


In the past 24 hours, have you had any of the following symptoms?

  • cold symptoms (i.e., cold, runny nose, sneezing, sore throat);

  • cough;

  • stuffiness;

  • increase temperature (up to 38 degrees Celsius);

  • fever (above 38 degrees Celsius);

  • loss of smell and/or taste.


Do you currently live with someone who has a fever?


Do you currently live with someone who has shortness of breath?


Have you had the coronavirus?

  • Has this been determined with a test in the past 7 days?


Do you live with someone or have been in close contact with someone  who was infected with the corona virus?

NOTE: close contact means that you spent more than 15 minutes with someone at a distance shorter than 1,5 meters.


And, have you had contact with this person, in the past 10 days, while he/she showed any of the complaints mentioned above?


Are you in quarantine because you:

  • have had direct contact with someone who was diagnosed with the corona virus?

  • Less than 10 days ago, you returned from a country or region with “code orange” or “code red”?

During these times, we are waiving our cancellation policy to clients with health complaints that might indicated infection by COVID-19. This means that appointments can be cancelled on the same day without having to pay for the session fee or scheduled to meet on zoom on the same day, if possible.
